Help to find and arrange support from care service providers

 

If you've had a social care assessment and you qualify for help from us, you may choose to let an organisation meet your care and support needs. This can be arranged by our brokerage team.

The team can look for a provider that can deliver a specific service in your home, or who will help you with your daily routine. For example, agencies that provide help with personal care and activities of daily living.

What brokers do

Brokers can help to:

  • Find service providers who can meet your needs and choices.
  • Give you information about service providers. For example, the cost of the service, CQC registration details, or whether they can meet your specific needs and preferences.
  • Arrange the service for you and have your support plan agreed by your social worker.
  • Give details about other types of support available from the statutory, independent and voluntary sector.
  • Provide details about a range of services, such as day services, home care, residential and nursing care homes.
